Varselsymptomer omfatter: feber eller andre infeksjonstegn, historie på skade, eller nylig spinalpunksjon, ledsagende allmennsymptomer, ferske vannlatingsvansker eller andre alvorlige eller progredierende nevrologiske utfall (som kan gi mistanke om epiduralabscess eller annen infeksjon, epiduralhematom, fraktur, cancer eller cauda equina syndrom). Ved mistanke om ukomplisert skiveprolaps eller ukomplisert spinalstenose er bildediagnostikk kun indisert etter 4-6 uker konservativ behandling og dersom kirurgi overveies.
